IMPORTANT: Before a student can participate in upcoming sports, a physical must be on file in the main office. Parent signature and clearance by a physician are required.

Athletic Physical Form

The Mission of the PHS Athletic Department is to provide opportunities for students to participate in a safe, coordinated, sequential interscholastic athletic program promoting participation, sportsmanship, skill and knowledge development.

One payment of the $50 Activity Fee will cover all sports and activities for the school year. Families who qualify for free or reduced lunch and fill out an household information survey will have TO PAY $25.00 PER SPORT - CAPPED AT $50.00.
